Percentage or ratio of free PSA/total PSA may be helpful in assessing prostate cancer risk in patients with mildly elevated total PSA (4.0 to 10.0 ng/mL) Percentage or ratio of free PSA/total PSA may aid in distinguishing between prostate cancer and BPH as a cause of elevated PSA. Percentage or ratio of free PSA/total PSA may provide risk stratification for men with prostate cancer. A lower free PSA/Total PSA may be indicative of a more aggressive form of prostate cancer (Carter 1997). Percentage or ratio of free PSA/total PSA increases sensitivity of cancer detection when total PSA in in the normal range.
