If your provider ordered services from us, you will receive an Explanation of Benefits (EOB) from your insurance company – this is not a bill. The explanation of benefits (EOB) is a statement of services. A separate patient statement will be mailed if a copay, coinsurance, or deductible is due on your account. Please contact our billing office with any questions regarding an explanation of benefits (EOB) or a received bill.
